Johnson and Johnson Medical Products Leadership Award
The Johnson and Johnson Medical Products Leadership Award was established to recognize an outstanding graduating student who is deemed to have made the greatest contribution to the advancement of the profession of nursing during his or her academic endeavors.

Terms of the Award
- The award is available to nursing students registered in a nursing school with current membership in the CNSA.
- The candidate must be a student in his or her graduating year at the time of application.
- The applicant must provide two letters of reference. One should be from a professor and/or a clinical instructor who can validate the information given on the application.
- The applicant must submit an official transcript of his or her marks to date with the application.
- The candidate must successfully complete the academic year in which the application is made.
- Current members of the CNSA BOD are not eligible for the award in their year of office.
Award Application Deadline and Instructions
Completed applications and supporting documents for this award must be received by the VPIER (and/or faxed to CNSA at 613.235.4476) by January 8, 2010. No applications received after this date or incomplete applications will be considered.
Application Forms
A completed application (created by the applicant) shall contain: 1. Name of applicant 2. Current Address and telephone number of applicant 3. Permanent address and telephone number of applicant (if different from current) 4. Name of the member school they attend 5. A current resume which includes:
a. a list of activities undertaken by the applicant during their post-secondary education as a member of CNSA, with emphasis on activities which exemplify the leadership qualities of the applicant or that the applicant has felt has led to the advancement of the nursing profession b. all employment and extra-curricular activities, including but not limited to CNSA, community organizations, and student government c. the name and description any other previous awards or recognition received
6. A personal statement identifying personal characteristics and leadership qualities of the applicant. This summary of no more than 350 words should include a description of what the applicant feels are their most significant contributions to the profession of nursing while a member of CNSA. It should also briefly identify their vision of nursing and their place in it. 7. Official transcript 8. Two letters of reference, as indicated in the terms of the award 9. The following statement, signed and dated; "I [name] agree to the terms of the Johnson and Johnson Student Leadership Award. I further believe all statements in my application to be truthful."
