The President's Award for Recognition of Exemplary Service

This award shall be presented annually to a past board member (no later than 1 year past their final term in office) who exemplifies the qualities of leadership, determination, teamwork, and the objectives of the CNSA. This individual can be nominated by any member of the Association.

Value of the Award

No monetary value will be assigned to this award. A certificate of recognition will be provided and the selected nominees name will be added to a plaque for display in the CNSA office.

Terms of the Award

1. The award is available to past board members who have completed their term within one year of the nomination.
2. Current members of the CNSA Board of Directors are not eligible for the award in their year of office, and members of the Board of Directors on which the nominee did serve their term will abstain from voting should they be members of the Awards Committee.
3. Individuals shall nominate one candidate only.

Award Application Deadline and Instructions

Completed applications and supporting documents for this award must be received by the VIER by November 10. No applications received after this date or incomplete applications will be considered.

8. The answers to the following questions, total answers not to exceed 500 words:
   a. How did this individual display the qualities of a leader?
   b. Give an example of when teamwork was displayed by this individual.
   c. Explain how this individual demonstrated determination in his/her term.
   d. How did this individual best meet the objectives of the Association?

Evaluation of the Application

The applications shall be evaluated by the three evaluators based upon the following criteria, based upon the applicant responses to the applications questions:

• How did this individual display the qualities of a leader? (0 to 5 points)
• Give an example of when teamwork was displayed by this individual (0 to 5 points)
• Explain how this individual demonstrated determination in his/her term (0 to 5 points)
• How did this individual best meet the objectives of the Association? (0 to 5 points)
